Assessment Instructions

For each statement, rate your organization. Use the guide below.

 1 = Not in place
2 = Needs significant improvement
3 = Somewhat in place
4 = Strong
5 = Fully developed and consistent

SECTION 1: PROGRAM CLARITY & IMPACT

  1. We have clearly defined programs based on community needs.

  2. We can clearly explain who we serve and how we help.

  3. Our programs have defined outcomes and goals.

  4. We collect data to measure program success.

  5. Our programs are consistently delivered and structured.

 

SECTION 2: FINANCIAL HEALTH & BUDGETING

  1. We have an annual organizational budget.

  2. Our revenue sources are clearly identified and tracked.

  3. We regularly review our financial position.

  4. We can anticipate financial gaps before they occur.

  5. We project our budget for the following year at least 6 months in advance.

 

SECTION 3: FUNDRAISING STRUCTURE

  1. We have a clear fundraising plan.

  2. We use multiple revenue streams (not just one source).

  3. We actively build relationships with donors.

  4. We consistently follow up with and cultivate supporters.

  5. Our fundraising is proactive (planned and intentional)—not reactive.

 

SECTION 4: ORGANIZATIONAL MESSAGING

  1. Our mission is clear and easy to understand.

  2. We can clearly explain the problem we address.

  3. We communicate our impact effectively.

  4. Our messaging is consistent across platforms.

  5. People understand what we do quickly.

SECTION 5: BOARD DEVELOPMENT & GOVERNANCE

  1. We have an active and engaged board.

  2. Board roles and expectations are clearly defined.

  3. Our board supports fundraising efforts.

  4. We recruit board members strategically.

  5. We have governance documents (bylaws, policies) in place.

SECTION 6: TEAM & LEADERSHIP CAPACITY

  1. Team roles and responsibilities are clearly defined.

  2. Tasks are effectively delegated.

  3. Staff or volunteers are supported and aligned.

  4. Leadership is not dependent on one person.

  5. We have systems for team onboarding and communication.

SECTION 7: SYSTEMS & OPERATIONS

  1. We have documented processes for key activities.

  2. Workflows are clear and consistent.

  3. Tasks are completed efficiently.

  4. We use tools or systems to stay organized.

  5. Operations do not rely solely on memory or urgency.
